HIPAA Notice of Privacy Practices

Your privacy rights and how your protected health information may be used

Effective July 2026

1. Our Commitment to Your Privacy

WealthHaus Diagnostics is an in-office lab collection site in Marietta, Georgia, providing professional specimen collection and mobile collection services. In the course of providing these services, we may receive or create protected health information ("PHI") about you. This Notice of Privacy Practices describes how we may use and disclose your PHI, your rights regarding your PHI, and our legal duties under the Health Insurance Portability and Accountability Act ("HIPAA").

Laboratory testing is performed by the applicable laboratory, which maintains its own Notice of Privacy Practices.

2. Our Legal Duties

We are required by law to maintain the privacy and security of your PHI; to provide you with this Notice of our legal duties and privacy practices; to notify you promptly if a breach occurs that may have compromised the privacy or security of your PHI; and to follow the terms of the Notice currently in effect.

3. How We May Use and Disclose Your PHI

Treatment. We may use and share your PHI to provide, coordinate, or manage your care — for example, collecting and preparing your specimen and transmitting it, along with the necessary order information, to the laboratory that performs your testing.

Payment. We may use and share your PHI to bill and obtain payment for services, including sharing information with health plans or other payers as needed.

Healthcare operations. We may use and share your PHI to run our practice — quality assessment, staff training, compliance activities, scheduling, and customer service.

4. Other Permitted Uses and Disclosures

We may also use or disclose your PHI without your written authorization when permitted or required by law, including: as required by federal, state, or local law; for public health and safety purposes; for health oversight activities; in response to a court order, subpoena, or other lawful process; for law enforcement purposes as permitted by law; and to avert a serious threat to health or safety.

5. Uses Requiring Your Written Authorization

Uses and disclosures of your PHI not described in this Notice — including most uses of psychotherapy notes, uses for marketing purposes, and any sale of your PHI — will be made only with your written authorization. You may revoke an authorization at any time in writing, except to the extent we have already relied on it.

6. Your Privacy Rights

Access your records. You may request to see or receive a copy of the PHI we maintain about you, including an electronic copy where applicable.

Request an amendment. You may ask us to correct PHI you believe is inaccurate or incomplete. We may deny a request in limited circumstances, and you may submit a statement of disagreement.

Request restrictions. You may ask us to limit certain uses or disclosures of your PHI. We are not required to agree in all cases, except where disclosure is to a health plan for payment or operations and the PHI relates solely to a service you paid for in full out of pocket.

Confidential communications. You may ask us to contact you in a specific way (for example, by text instead of a call) or at a specific location. We will accommodate reasonable requests.

Accounting of disclosures. You may request a list of certain disclosures we have made of your PHI.

Breach notification. You have the right to be notified following a breach of your unsecured PHI.

Paper copy. You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.

7. Privacy Complaints

If you believe your privacy rights have been violated, you may file a complaint with WealthHaus Diagnostics using the contact information below. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

8. Changes to This Notice

We may change the terms of this Notice, and the changes will apply to all PHI we maintain about you. The current Notice will always be available at this page with its effective date.
